University of the Cumberlands Information Governance Patient Insurance PPT Need 3 page PPT, please look into the attachments before you bid.The ppt has to be similar to the one in the attachments. In this paper, the focus is on Patient Insurance, and to achieve this. We shall do a records inventory for the area of patients’ records, individual staff records, ward records, equipment and facilities records, and claims records (Smallwood, 2019). We will be surveying and interviewing staff attached to the Emergency Medical Services Department, Accounting department, and the IT department in City General hospital.Record types that we will be hoping to find and peruse through in the Emergency Medical Services Department are as follows: Patients clinical record Individual staff records Ward records Administrative records of educational value To provide clarity on how to conduct the retention and disposal of the records, we shall use a Records Inventory Survey Form to collect all the necessary information about the location and identity of all the records held by the Emergency Medical Services Department. We shall equally use this survey to establish which other department has access to these records. We will employ a physical survey to since it is relatively more reliable, accurate, and provides detailed information. Below is the records inventory form: Inventory Questionnaire Emergency Medical Services Department, City General Hospital Question Yes No N/A Remarks How precise or accurate are your Inventory records? How exhaustively does your department meet Customer demand? Are there policies to govern proper accounting, reporting, and protecting the inventory? Are these policies and procedures up-to-date and clearly communicated? Are the receiving, issuing, accounting and storing duties differentiated? Are there adequate provisions to safeguard against loss, damage or theft of items? Are there Access restrictions for authorized personnel only?Does management monitor and approve the write-offs of obsolete and inactive inventories? Are there procedures for billing and issuing linked to ensure all items are billed? Is substantial Insurance coverage provided?Does the department generate: Organization chartsAnnual reports Do procedures for physical count provide for: Adequate supervision? Clearly marking unusable and dead inventory? Are there other records shared with other offices or stored in other places other than in the department filing area? What electronic systems are used in the department?Does management monitor and approve the write-offs of dead and unusable inventories?Do departments compare and balance received against receiving reports? Are discrepancies between physical counts and existing records investigated and resolved? The Survey questionnaire is meant for the chief finance officer (CFO), the Information technology (IT) manager, and chief Store Keeper. Also, to take the survey will be staff working in these departments. The three have been chosen to receive the survey since they are directly involved in all stages of the Inventory cycle. These survey questions were designed to assist in establishing the risk factors that could lead to mistakes, inconsistencies, corrupt dealings, or illegal practices within the Emergency Medical Services Department.Following are questions that will be used as follow up to the initial survey:a. Are you likely to participate in another survey in the future?b. What areas do you think were left out in the survey?c. Did the survey questions adequately cover the issues surrounding the inventory?d. Tell us one thing you think was unimportant in the surveyRecord retention schedule control records’ life cycle (Guidance, 2020). This entails preserving or destroying the records. The records that will be retained for five years or more include: include a list of individuals with essential health and safety duties, health and safety policy, risk assessments already performed, machinery and equipment maintenance records, employment records. Claims’ records can be retained up to 3years after the incident, same as records for injuries, disease, and accidents. Records of employees exposed to harmful biological agents should last for ten years, while those with information on exposure to hazardous substances and asbestos should be kept for over 40 years. Records that are not meant to be retained should be destroyed permanently. The purpose of this is to protect the department and its staff by preserving confidentiality. To ensure proper destruction, we shall use the “cross Shredding” (cutting the paper into squares of ¼ inch each), or we could do “pulping” which involves turning the paper back into pulp. The inability to capture the information necessary for required reporting from electronic health records is one of the significant problems. This can be mitigated by ensuring the City Hospital is analytics-driven. Secondly, several serious errors that adversely affect patient’s safety resulting from inadequate user training on how to use the system, as well as the laziness of staff in documenting and charting can be solved by becoming nimble enough to respond quickly and accurately to information requests related to patient care and diagnosis, legal holds, e-discovery and litigation, and for accountability and auditing. Lastly, the need for better security of protected health information to comply with stricter regulatory requirements can be solved by developing processes to ensure better coordination of care throughout the hospital.References Guidance, S.
