Andrea is a 29-year-old Caucasian woman employed as
the codirector of development (aka chief fundraiser) for
a large nonprofit organization in the United States, a
national charity that funded countless local efforts to
support at-risk children and their families. Her job
required her to travel to most of the major cities east of
the Mississippi River, often several times per year, to
meet with both established and potential donors as well
as oversee the major fundraising events being
coordinated by the local offices. Andrea was proud of
her career success and very invested in and gratified by
the humanitarian mission she served. Likewise, she also
placed a high value on staying connected to her close-
knit family; thus, she lived only 20 minutes away from
her parents’ home in the DC suburbs and usually saw
them at least weekly in spite of her intense travel
schedule. She typically enjoyed a sunny disposition and
was outgoing and optimistic by nature, all of which had
been major assets in her chosen profession.
Earlier in the year, Andrea had become engaged to
marry a man named Shaun whom she had met at one of
her fundraising events and had been dating for more
than 2 years. He also had a demanding job working as a
top aide for a U.S. government official, which likewise
required long hours and significant amounts of travel.
From the beginning of their relationship, he had been

very open with Andrea that he had political aspirations
of his own, a fact that Andrea embraced and admired.
They had a lot in common, deeply enjoyed each other’s
company when they had time together, and found
satisfaction in viewing themselves as a power couple
of sorts. They both viewed themselves as approaching
the prime decades of their lives closely bonded to their
joint and individual aspirations, so it was rather
surprising to both of them when Andrea reluctantly
began to complain to Shaun that she feared she was
slowly but surely losing [her] mojo. She had noticed
that she had begun sleeping poorly, often waking
between 3 and 4 a.m. unable to return to sleep, causing
her to feel tired and clamoring for a nap that her
schedule never allowed. She began to have difficulty
with focus and concentration to the degree that
responding to 50-plus e-mails every day became an
onerous chore in a way that felt different and
demoralizing. Her previously effortless effervescence
began to feel artificial and labored to the degree that
she began turning down invitations from friends and
avoiding social media. And most saliently, Andrea
noticed that on most days she felt quite sad and morose
in a manner that was “totally out of character and for
no apparent reason. She confided to a close friend, “I
feel like I’m walking around like someone in my family
suddenly died, when in fact nobody has.

After struggling with these difficulties for 2 months and
realizing something was really wrong with her,
Andrea made an appointment to see her primary care
provider (PCP). To both her relief and disappointment,
all of the tests came back normal, and her PCP
suggested that she suspected she was experiencing a
depressive episode. Andrea felt relieved that she wasn’t
suffering from something like leukemia or some other
illness that could explain her symptoms of utter
exhaustion, but she also felt disappointed that it wasn’t
something that a regimen of vitamins couldn’t cure.
Her PCP offered her a prescription for antidepressant
medication, which she accepted but did not fill at the
pharmacy due to feeling certain her depression was due
to stress at work, which had considerably increased
during the past year. Instead, she decided that she
wanted to take a natural approach to treating her
depression symptoms. Accordingly, she attacked the
problem in the same direct manner she approached
most things: renewing her efforts to exercise and eat
healthily, seeking the support of friends, and most
important, making adjustments at work by successfully
lobbying to hire a second executive assistant to whom
she delegated some of her travel-related
responsibilities, allowing her to reduce her own
traveling commitments considerably.
Although it required some patience on her part, Andrea
slowly but surely began feeling better. Later on, she

Although it required some patience on her part, Andrea
slowly but surely began feeling better. Later on, she
would report to her psychologist that her sleep pattern,
energy level, and mood gradually all returned to normal
during the following 4 to 5 months, so that when fall
arrived, she was able to fully plunge herself into all of
the countless smaller details needed to plan the kind of
early-summer wedding she had hoped and expected to
pull off for herself and Shaun the following year.
Before she knew it, she had begun burning the
midnight oil, giving her career her full attention and
effort during the day, and then working on her wedding
and honeymoon plans from 9 p.m. to 1 a.m. and later
more nights than not. No detail was too smallfrom
the various fonts she considered for her invitations,
tracking down and evaluating the merits of 10 different
varieties of roses that would be available in the early
summer, to trying to accommodate the numerous food
allergies and sensitivities plaguing some of her guests
by constructing a menu that the caterer complained
turned into an assortment of dishes that altogether
seemed odd. Andrea would admit that she was allowing
herself to be consumed by the planning, but
simultaneously insisted to her fiancé and others, who
were becoming a little concerned, that she was having
a ball and loving every second of it.
Unfortunately, 10 weeks or so prior to their wedding

Unfortunately, 10 weeks or so prior to their wedding
day, Andrea, along with the wedding plans and
logistics, began to unravel. Shaun first noticed that
Andrea seemed to be sleeping less and less. For the first
week or two, he convinced himself that it was not a big
deal, that she was merely excited about the wedding,
which was to be expected. He discerned that it was not
that Andrea could not sleep as much as she seemed to
need very little sleep, which was quite different than
when she had felt utterly exhausted but unable to sleep
the year before. In response to his bidding, Andrea
always agreed to lay down for 3 to 4 hours, but he
suspected she had not slept more than 1 or 2. When
Shaun expressed concern about this and the fact that he
thought she was losing too much weight from not
eating enough, Andrea insisted she felt amazing, and
this indeed seemed trueshe was even more upbeat
and energetic than usual, actually more buoyant than
he’d ever seen.
At the same time, it had gradually become apparent to
both Shaun and Andrea’s boss that in spite of her high
energy level, her daily efforts were steadily becoming
much less focused. On one recent morning, the couple
got into an argument when Andrea abruptly decided
that she needed to expand their wedding invitation list
by 50% owing to Andrea’s burgeoning belief that they
were at risk of offending countless people by not
including tham However the list included nennle lilza
