Review any previous literature review you have completed related to your CGE project topic. If you have not previously completed a scholarly literature review on your topic (i.e., your topic may have changed), then you will create a new literature review with this assignment. Complete a database search on your project and compile the results, adding any new or relevant scholarly sources to your review. Focus on the most relevant articles or studies that address your problem or concern or discuss resolutions to the problem. Include any studies that do not support your projected resolution or take a different approach. In addition, search for systematic reviews about the problem or concern. Write a review of the literature or update your existing literature review using the following outline.

PICOT Worksheet—Week 1

Purpose: To identify a problem or concern that nursing can change and develop a PICOT question to guide the change project
PICOT Question | |
| What is the PICOT question? Does expanding the ER bed capacity and increasing the ER staff as compared to the normal ER operations result in a reduction in the Left Without Being Seen (LWBS) rate from 4.6% to less than 2% in the ER in a period of one year? Define each element of the question below: P (patient, population, or problem): Patients seeking services in the ED
| |
| What is the practice issue or problem? What is the scope of the issue? What is the need for change? The practice issue identified is the Hospital Emergency Department (ED) Left Without Being Seen (LWBS) high rate, which is calculated at 4.6%, and the national benchmark for LWBS is 2.0%. According to Li, Brennan, Kreshak, Castillo, and Vilke (2019), LWBS represents a significant safety and quality concern. As such, the LWBS rate is often used as a performance metric for analyzing the performance of the ED. Patients leave the ED for a variety of reasons: even some who have already been registered and seen or assessed by the triage nurse (Saia & Fonzo, 2017). A low LWBS, therefore, is a reflection of the efficiency with which a particular ED achieves its main objective of being the community’s safety net (Mataloni, Colais, Galassi, Davoli & Fusco, 2018). Patients who are LWBS hurt the Hospital ED. The concern about patients who leave without care is twofold: it results in a financial loss for the facility, and it causes increased exposure to liability for the patients who left without care (Mataloni et al., 2018). As an example, the average financial loss for the facility was calculated to reach $1,345,791.00 yearly. There is, therefore, a need for an intervention to help reduce the LWBS rate to an acceptable level to lower the probability of financial loss and liabilities associated with patients leaving without care. | |
| How was the practice issue identified? (check all that apply) X Safety or risk management concerns X Unsatisfactory patient outcomes _____ Wide variations in practice X Significant financial concerns | ___ Difference between hospital and community practice X Clinical practice issue a concern X Procedure or process a time waster _____ Clinical practice issue with no scientific base _____ Other: |
| Search terms: How can you narrow the search? · Left without being seen · Consequences of patients who left without being seen · Financial losses resulting from left without being seen | |
