Hip Replacement Surgery and Recovery Case Discussion please I need about 500 words and 2 science referencesI have attached the clinical case assignment instructions and two examples Clinical Case #2
64yo female presents to office with a 5 year history of progressively worse right
hip pain. The pain radiates into the patient’s groin and proximal thigh. Pain has
progressed to the point where she requires a cane and has difficulty performing
ADLs (activities of daily living). No relief with activity modification or NSAIDs
(non-steroidal anti-inflammatories). She subsequently underwent surgery.
Presented are her 6 week postoperative X-rays. Provide a short synopsis in the
discussion board forum of the patient’s pathology, how that pathology
contributed to her symptoms and eventual treatment.
*Clinical Cases are optional/ungraded and are provided solely to enhance students’ educational
experience.
A/P Right Hip
Upon reading the problems and age of the patient who came in with progressively worse right hip pain
and seeing the postoperative X-ray, I automatically knew it was a total right hip replacement. Being that
the patient is over the age of 50 would lead me to believe this was brought on by age related wear and
tear arthritis better known as osteoarthritis. Osteoarthritis is were the cushioning cartilage of the bones
pertaining to the hip deteriorate. Which then causes the bones to rub together causing pain and
stiffness. Osteoarthritis can severely interfere with your day to day activities even simple ones like
putting on your shoes. This patient stated her pain had progressed to the point of her requiring a cane
as well as difficulty performing daily living activities; even when provided non-steroidal anti-
inflammatories and activity modification. So, I believe after all other treatment options were offered
and she tried them with no success a total right hip replacement surgery was suggested. In a total hip
replacement surgery, the cartilage and bone that are damaged are removed and replaced with
prosthetic pieces. “The patients damaged femoral head will be removed and replace with a metal stem
into the hollowed center of the femur. The stem will be “press fit” into the bone or cemented. Then a
ceramic ball is placed on the upper part of the stem. This replaces the damaged femoral head that was
removed. After you remove the acetabulum, which is the damaged cartilage surface of the socket. You
replace it with a metal socket that is either screwed or cemented in to hold the new socket in place.
Lastly
, depending upon the surgeon and patient choice a plastic, ceramic or metal spacer is inserted
between the new ball and socket to allow for a smooth gliding surface.” (Health Jade Team) After this
surgery is complete most are encouraged to use the new joint immediately. Many patients are enrolled
in a physical therapy or exercise program following their new hip replacement. This is suggested to the
patient to help regain and improve the use of there new hip joint. Majority of patient’s state that the
performance of daily activities is more easy and comfortable able hip replacement surgery.

Progressively worse hip pain in a woman of 65 years could be from multiple converging factors. We do not have data
on any previous injuries to the hip, but these are possible factors in the deterioration of the joint and radiating pain;
1. Previous impact injury to hip that set-in play an arthritic dynamic over years.
2. At the age of 65 we most certainly see a woman of menopause. Depleted Estrogens and Progesterone (most
probably testosterone as well), set in motion dysfunctional bone metabolism. Osteoclastic and osteoblastic
modeling is hindered. Other variables would be the levels of vitamins D, K2 and Ca+ and general mineral
imbalances. Osteopenia and full-blown osteoporosis are very common in women post menopause for these
reasons.
Gradual deterioration of the hip and pain signaling to the brain will tend to shift weight off the favored hip/leg thus
changing the balance and mechanics of the body. Pain and stiffness of the joint creates negative impact on QOL and
ADL.
One can have deterioration of one surface or multiple surfaces along with capsular soft tissue erosion creating
instabilities and pain.
The surgery involves removal of the femoral head, replacement with a prosthetic head on a shaft inserted into the femur
and reaming of the acetabulum which is then lined with a bowel shaped synthetic joint surface for smooth gliding
action. There are variations of the procedure such as a partial replacement which is used in femur fractures (displaced).
The surgery is truly liberating for the patient and recovery is relatively fast in todays procedures. I have several friends
that have undergone total hip replacement with very fast return to ADL and QOL was improved dramatically. I myself
am the proud owner of a total right shoulder replacement (2014) and the results have been nothing short of amazing.
Almost 100% normal function has been returned and zero pain.
