FRAMEWORKS FOR OUTCOME EVALUATION

This week we will continue to explore frameworks for outcome evaluation and quality improvement of population health efforts. This week you will contact and interview the population health/quality improvement director where you work, then answer the following questions: 1.) Can you please provide a brief description of the population demographic the system serves (inpatient and outpatient)?; 2.) What do you see as the top five health issues/problems of your population?; 3.) Can you please discuss some of the population based health strategies that our system has used or is currently using to address these health issues/problems?; 4.) Can you please describe the outcomes of these activities and if you have changed any approaches based on outcome data? Is so please provide an example of a change in approach; 5.) In what ways can advanced practice nurses (CNSs, CRNPs, CRNAs, and CNMs) contribute to the success of population health based interventions within the system?
