Total Points: 29.0 Part 5: PROJECT EVALUATION (FINAL PROJECT PAPER)
Submit during Week 10
DEADLINE: July 14th
IMPORTANT: Please be sure to receive feedback on your previous submissions
(Parts 1-4) BEFORE you begin Part 5 (your project evaluation/final project paper.
The purpose of Part 5 is to bring all the information you have collected and learned
into one report. Remember, you will NOT be evaluated in terms of your success in
achieving your goal. Emphasis in grading is on your understanding and analysis of why
you were or were not successful.
1. Summary of health plan: Summarize your health plan that you described in the
Part 1 submission. Include your (a) issue statement, (b) goal statement (be sure
to describe how you changed your issue statement that may have initially been
negatively worded to a positive goal statement with all three required
components), and (c) 50% goal statement (describe how you applied the 50%
rule).
2. Health behavior model/theory: Describe your health behavior change
experience in relation to the concepts covered in relevant health behavior
theories/models. Briefly describe the theory you chose in Part 2. Did the theory
best explain your health behavior change? Why or why not?
3. Summary of baseline observation week: Summarize your baseline performance
of your health behavior. Dont include daily reports, but rather summarize your
overall trends and/or averages for the baseline observation period.
4. Weekly summary of behavior change journal entries: This area should include a
weekly summary of your daily journal entries and your weekly progress notes.
(e.g., My weekly journal entries demonstrated
or My weekly progress
notes showed that I was
). Again, do not just copy and paste the daily diary
entries. Rather, look over your diary entries and include a summary of your
weeklong behavior change periods.
5. Experience of trying to accomplish the goal: Describe your overall experience
of trying to accomplish your health behavior change goal. (Dont copy and
paste from journal entries. What is your overall reaction to your behavior
change project?)
6. Summary of information from research sources: Summarize the information you
learned from each of your research resources.
7. Analysis of thoughts/behaviors that would help: Reflect on the thoughts and/or
behaviors that you thought would help you achieve your goal. (See Part 2) Were
they actually helpful in reaching your goal? (See Part 4) If so, how? If not, why
not? What other thoughts/behaviors were helpful? (See Part 4)
8. Analysis of power of saboteurs: Provide an analysis of your thoughts and/or
behaviors that you thought would sabotage you in achieving your goal (See Part
2). Did they actually sabotage you? (See Part 4). If so, how? If not, what other
thoughts or behaviors turned out to be your saboteurs? (See Part 4).
9. What you learned about yourself: Describe what this experience taught you
about yourself. To do this, you may want to reflect on your saboteurs and the
power they had over you. Also, consider the effects of acting on the
thoughts/behaviors that you believed would help your progress toward your
goal. Did the small rewards help you, or did you consider them “silly” and not
take them seriously?
10. What you learned about health behavior change: Discuss what you learned
about making a health behavior change. Identify a link between what you
learned in this course (through lectures, readings, and activities) and your
experience in completing this project.
11. Statement of future intentions: Do you think you will try another health
behavior change in the future? If yes, why? If no, why not?
12. What you would do differently next time: Did you find anything you might like
to do differently when trying to change health-related behaviors in the future?
13. Value of this project: Overall, was this project worthwhile to you? Why or why
not?
Formatting Your Part 5 Project Evaluation (final paper):
Because this phase requires you to reflect on your previous submissions and diaries,
please be sure to save all of your previous work throughout the semester. If you
received any corrections/feedback, please do not forget to incorporate them into
your final submission (Part 5) so that you will not get points taken off twice!
The required components for Part 5 Project Evaluation Final Project Paper are as
follows:
(a) Title page per APA style (http://owl.english.purdue.edu/owl/resource/560/01/)
(b) Your entries on Part 5 items described above (dont forget to incorporate
corrections based on previous feedback)
All these components MUST be integrated into one MS Word file.
It is greatly appreciated when students try to make the grading process simple. That
means that all the important components are easy to find under bold headings in an
organized paper. Please be sure to use the template provided and follow the grading
rubric when you organize your paper.

See below for the rubric we will use to grade your assignment. The first column is a
list of the different areas you will be graded on, the second column shows the points
