University of Phoenix Supervisor of Quarter Award Analytical Review Please understand that these nomination forms should “paint a picture” of what you want the committee members to know and realize about the candidate you are nominating. This form will be the only testimony of the nominee that we will have in front of us to use when scoring so in all fairness it will be solely up to you, the one nominating the supervisor/employee, to express to the committee “why” your selection should receive the highest score. There is no limit as to how much you may write in these nomination forms. Under each numbered sentence, you may type as much content as you need to, without worrying about the format of the form; just keep typing and the next numbered sentence will shift down. (I’m mentioning this as an answer to a previous question concerning the length/limits to write-ups). In your write-up, be as detailed as possible in all areas, ie. Patient-Centered, Mission Support, Self-Development, Character, work ethics, professionalism, team building, contribution to moral, above and beyond accomplishments, etc. etc. You should tell us the WHO, WHAT, WHEN, WHERE, WHY, and HOW MUCH as you’re painting your picture of the nominee. The more details you provide, the better the committee will understand your candidates knowledge, skills, abilities, and potential. Think of quality and quantity in every sentence. Writing may not be everyone’s strong point, and that is totally understood. However, in order for the committee to score nominations high we will have to be presented with your “best” write-up.

****Here’s a little background on this project. I want to nominate my supervisor for an award, that he really deserves, but I need a bit of help answering the questions. He is your typical ideal leader. He is a team player, who never hesitates to help his employees when they are struggling. He is very organized and detailed. He went ahead and planned and organized the rebuilding of the Reproduction department within a week (making sure the engineers did the electrical work for the new machines being installed, made sure the painters painted the new room, made sure the EMS department cleaned the floors, and made sure the ITs installed all the computers). During this Pandemic one of his employees tested positive with the virus and he made sure everyone in his department got tested and was taken care of, even though they were falling behind on work.*****
