You are a nurse in a step-down unit caring for a new patient who was transferred this morning from the trauma ICU. He is 36 years old, married with two small children, and no medical history. He is recovering from a recent motor vehicle accident which he had several surgeries for initially (explorative abdominal laparoscopy, splenectomy, and stabilization of fractured left femur and right humeral bones with internal fixation).

He is getting in-hospital physical, occupational, and nutrition therapy. In your assessment, he is AAOX4, stable, one-assist with ambulation, pain well-controlled, and surgical sites healing well. His appetite is fair (consumes <50% of meals). He is still waiting for the appropriate braces and ambulation equipment to be delivered from the medical supply company. Orders are to progress diet as tolerated, participate in ordered therapies, and evaluate the patient for potential rehab vs home with home health care.

He tells you during the am assessment: “Nurse, my wife has to go back to work, she’s already missed too many days because I wrecked the car! I have insurance, but I am still going to owe bills and we are on a budget. My mother is watching the kids, but I need to be there! When can I go home? I can’t sleep and I don’t feel like eating. I need to go as soon as possible. I might lose my job, and I need to support my family. I’ve never been through something like this, when can I be discharged?”
