Coding Scenario and Dilemma – (Assume that the codes and RVUs are current and accurate for the purpose of this assignment.)

Aimee is an outpatient day care surgery coder at LiveWell. As she begins her work, she gets a call from surgeon, Dr. Orlando. He says “Calling to tell you that I did a procedure Tuesday on Vixie Padrome, account 0328196, and I want you to use CPT code 21554 on it”. You tell him that you will look up the account and get it coded today. You locate the account, review it and make the following notes:

Procedure Name on Vixie’s Operative Note: An excisional biopsy of enlarged lymph node measuring 5.1cm on the left side of the neck.
Code for the above procedure: 38510 (biopsy or excision of lymph node, open, deep cervical node.
Code per Dr Orlando: 21554 (excision, tumor, soft tissue of neck, greater than 5 cm)
Dr. Orlando’s operative note does not mention or contain the word ‘tumor’
RVUs (Relative Value Units) for 21554=8.91; for 38510=6.69
Called Dr Orlando, explained the correct code and he was insistent that since the procedure was time consuming and invasive, the lymph node should be coded as he instructed to 21554.
