Learning Outcome: To develop a written proposal utilizing the GNOME criteria for a focused assessment analysis, to be implemented at clinical setting. The topic should be of interest to the student and benefit to the organization/clinical site. Focused Assessment Analysis should be developed in consultation with the preceptor and course faculty. The purpose of the project is to demonstrate integration of concepts, research, and advanced clinical skills and knowledge by improving the design and delivery of health care for the identified patient/family/group. The project should address a need in the clinical agency while also meeting the student’s learning objectives. GNOME proposal will be the foundation for the Focused Assessment Analysis Paper. Method: Each student will prepare a written proposal in the GNOME format. GNOME stands for Goals, Needs, Objectives, Methods, Evaluation. Proposal should be no more than 1-2 pages in length.   Evaluation criteria:   Goals: identify /state a policy initiative related to Healthy People 2020 (or another health policy related to your area of expertise or future CNS role), improved patient outcomes, quality improvement, or safety and relate your project goals to this larger health policy. Include in goals A: goal(s) of project as it relates to a higher health policy, and B: goal of student e.g. Capstone/course project. Include APA citations. (1 point). Note at start of Fall 2030 – HP 2030 has a delayed implementation.   Needs: What are needs of your target audience (population)? (Clearly identify who [group] the assessment tool is utilized on). What if anything does the literature have to say about the tool and organizational need regarding the goals for your project? What does the literature say about the problem of the population addressed by your Focused Assessment Project, (epidemiology, risks, rates of death or morbidity associated with the problem), Relates to Goals. Include APA citations (2 points).   Objectives: Demonstrate the ability to write Bloom’s objectives in the cognitive, affective, or psychomotor domain. Objectives are written in measureable terms focusing on what will be evaluated as evidence of learning outcomes. Relates to identified need(s). (2 points).   Methods/Analysis of Evidence Methods: are clearly identified, fit the content, and facilitate the measurable learning outcome objectives of the learning. Add/utilize citations from the literature to support the methods as effective in providing assessment to population identified. Consider – what evidence supports the use of these methods (literature review, analysis of evidence), and cite the reference for each method. Provide appropriate APA citations for your references. Relates to objectives. (2 points).   Evaluation Methods: are clearly identified and are appropriate to evaluate the assessment tool intended. Add citations for each evaluation method utilized – with the literature support the methods are seen as effective and trustworthy. What evidence supports the use of these evaluation methods? Relates to your literature review methods. Include project, recommendations for practice and dissemination methods. Provide appropriate citations for your references, utilize APA format (2 points).   Citation and Referencing: APA 6th edition formatted reference list (1 point). Include level of evidence utilized (listed on grade rubric).
