CHAPTER I: INTRODUCTION
The Veteran Affairs department is critical in the United States of America. It exists at the
cabinet-level agency. The primary purpose of the department is to offer comprehensive services
regarding healthcare to the military veterans that are legible (D’Avolio et al. 2012). The
provision of healthcare services takes place at the outpatient clinics and veterans affairs medical
centers, which exist in various locations of the country. Veterans have a myriad of facilities that
they can chose from depending with their preferences and locations. There exist non-healthcare
assistance that the veterans receive from the department. They include support in education, life
insurance, compensation for disability, home loans, holiday rehabilitation and even burial
services (Kang et al. 2000). These services are offered to the veterans as a way of appreciation
for their services to the country and recognition of their commitment to the country. The
department may also provide memorial services to members of the fallen soldiers in the over one
hundred and thirty-five national cemeteries located in various parts of the country. It is worth
noting that the United States government has always given benefits to military veterans since
time immemorial (Kizer et al. 1997). However, it was not until 1930 that this department got
established. The idea behind the establishment of the department was to come up with a
centralized department on government that would be able to respond to the needs of the veterans
within reasonable time. The government came to the realization that many Americans were at
one point or another affected by a family member that served and as such, the veterans were
important members of the society .Time went by, and in 1982, the department resorted to having
an extension of its services to the non-veteran members in cases of emergencies and unprepared
predicaments.
9
The Veterans Department always has a share of the national budget. The department is
known for offering employment to approximately 388,000 people in its various medical centers
throughout the U.S. The Secretary of Veterans Affairs heads the department and is appointed by
the U.S. president with the Senate’s consent. Civilians can benefit from the VA when they are
involved in severe disasters declared by the president. The department has a legal right to offer
paid medical services in emergency cases as well. The U.S. Department of Veterans Affairs,
along with a host of other benefits, provide housing assistance to veterans that are homeless. The
department grants more than $29 billion yearly in disability benefits to military veterans.
The Veterans Department has three main subdivisions. The first category is the Veterans
Health Administration. The Veterans Health Administration’s role is to offer healthcare services
to all veterans in the form of medical, dental, vision, long-term care, counseling, etc. It also
carries out biomedical research (Perlin et al. 2004). The Veterans Bio-Medical Research Institute
(VBRI) work in partnership with the VA, Department of Defense (DoD), National Institutes of
Health (NIH), and the National Association of Veterans’ Research and Education Foundations
(NAVREF) to advance Veterans’ health by maintaining medical research and education
programs for veterans. The second department is the Veterans Benefits Administration, which
carries out the initial registration of veterans, to determine their eligibility for enrollment.
(Gilman and O’Hara, 2018). The final subdivision is the National Cemetery Administration,
which offers the memorial services for veterans, provide burial benefits, and maintain the VA
cemeteries.
Background
The Veterans Affairs Department has always had a share of the national budget. It is
considered a vital organ in the administration of the United States government. Over the years,
10
there has always been a comparison between VA healthcare and private care. Some military
veterans have opted to go not only go VA healthcare but also for private care. There are many
underlying issues that give rise to high expenses. The United States has always wanted to
appreciate the service of military personnel offer to the country, and one way to do so has been
through the Veteran Affairs department. However, it takes a lot of consideration and crossexamination for one to enroll in the services offered by the department. There are various aspects
in terms of bureaucracy exhaust people in their efforts in gaining services from the VA or
receiving it from private care. Many of the deeply rooted problems cloud the noble intentions of
the VA in providing the much-needed health care to veterans. Many of the veterans who go for
the services provided by the Veterans Department have often given unsatisfactory comments and
feedback suggesting that the problems experienced are too many to be ignored. It calls for a
process that will create effective remedial actions so the department can focus on the noble cause
for which the department was formed to address does not end beginning again. It is the hope that
many of the challenges will be identified and resolved, so that many of the veterans can continue
seeing the VA as their preferred option for seeking their medical care.
Statement of the problem
The VA health care was established with the intention of ensuring that the veterans had
access to safe and high-quality medical care. Over recent years, there have been research done to
establish the effectiveness and efficiency of the Veterans Affairs Department. Even after the
research, scholars have found the existing gaps in the reports. Few cost analyses has been done
to establish the department’s effectiveness. The comparison between private care and VA
medical care has not been fulfilling. There is a need to determine whether the VA medical care
gives full support to the retired military veterans, while cutting on the government’s costs or if
11
the medical care prevents these military personnel from getting better healthcare services using
VA resources. The government invests a lot in this department, as it is an essential aspect of
governance in the military. However, studies show that veterans do not think they receive the
best healthcare from VA. These veterans feel they can get better medical care outside the VA,
but they cannot afford the cost. Many of the veterans have also expressed their concerns about
the department’s leadership asserting it has ignored their needs and remained quiet despite
several reasons. Veteran have expressed the departments need for change in areas of employees,
resources offered, time spent, compassion shown, appointment systems, etc. The leadership will
also form an important source of information other than the veterans to ensure the findings are
unbiased. There is a need to establish whether their feelings are true or false. After this study,
they shall make up their minds concerning the issue. The department has also employed many
people, in an effort to decrease the workload and offer more services to veterans. These
employees receive salaries from the government’s budgeted amount. However, the government
spending is high, yet some veterans still find it difficult to access these services. Some of the
veterans have resolved to stick to private medical care. The considerable spending comes about
with fewer returns. This research will address this issue by having an in-depth cost analysis.
From the results, the veteran personnel will have to make tough decisions regarding what they
want, whether to stick to the VA’s healthcare system to find out private care using their
resources. For every problem, there must be a long-lasting solution. For this problem, the
solution shall depend on the results of the study and the perception that the military veterans
shall adopt regarding the Veterans Affairs department.
12
Study purpose
VA healthcare system was created with a noble intention of ensuring that veterans had
access to medical care of the best quality. Numerous reports suggest that the medical care fund
disbursed to the veterans had been misappropriated together with excessiveness and spending
deemed contradictory. Another major drawback for VA medical care has always been the
constant accusation the department faced with a massive backlog of patients that need to be
attended to. Another drawback is inadequate heath care services. The reason for this research
work is to carry out a cost analysis regarding private care and medical care. It shall establish
whether the VA healthcare system hinders the military veterans from accessing some medical
services or if it gives support to the veterans, while saving government resources. It shall also
undertake a review of the system to establish whether it is effective or whether it is the primary
cause of veterans seeking alternative private care as opposed to going for the Veteran medical
care. The vital tools shall be mathematical calculations of the governments’ expenditures and
services costs the veterans receive on an annual basis. It shall include an in-depth cost versus
benefits analysis to establish whether the veterans get the value for their money. The preference
of the veterans’ type of medical healthcare will be obtained from interview sessions with them.
The research method will be quantitative in this context. The data sources will be the statistics
posted on the Veterans Affairs website. The annual budgetary allocations are also another critical
data source. Questionnaires and one-on-one interviews shall also be vital sources of primary
data. The participants in the research will be individuals that hold administrative positions in the
department, some of the employees, military personnel, government officials and mathematical
professionals. The geographic location of this study is the United States of America.
13
Conceptual framework
According to Chen, any input requires an output (Chen et al. 2018). The output can get
measured by using various metrics. A comparison of the output with other aspects regarding the
input is also vital. In this case, the input is the government resources in the Veterans Affairs
domain. The output is the expected healthcare services the military veterans should receive from
different healthcare centers distributed across the country. The comparison aspect comes in
between the services received from the department and the services received when using the
private resources. It is from this theoretical framework that the research will provide a logical
research process.
Quantitative research is also vital, where large numbers are in comparison (Goertzen,
2017). The expected numbers of comparison are the extensive government resources, the cost of
services veterans receive from the department, the cost of the private healthcare and the overall
efficiency of the two approaches.
There are five principles the research shall be based on. The first principle is minimizing
risk. The study shall be undertaken with minimum risks without exposing the respondents so
much. The purpose of the questionnaires will be made clear to the respondent, bringing in the
second principle of informed consent. The respondents’
details shall not get exposed, as an aspect of the third principle of confidentiality (Padulo et al.
2018). There shall be no deceptive practices. Everything shall be apparent. In the research
course, there shall be a right of withdrawal. Anyone will not be under pressure to get into the
research up to the completion stage.
14
Research questions
There are specific research questions for this body of work. Answering these questions
would provide a solution to the problem statement. The research questions add weight to any
study’s objectives. The research questions include:
1. Does the Veterans Affairs medical healthcare system fully consider the needs of the
military veterans?
2. Is it more cost effective and beneficial for veterans to receive care outside of the VA?
3. Does the VA save the government from spending a extensive amounts of money and
resources?
4. Does VA prevent veterans from obtaining improved healthcare services on a private
basis?
Nature of the study
The study will involve a methodology where data shall get collected from various
sources. There shall be interview sessions with the veterans, government officials and VA
department employees. There shall be printed questionnaires to get a conclusive response from
the target population. There shall be no revealing identification of the respondents. Respondents
will be able to speak their minds conclusively. After seeking consent from the veteran
participants, adequate care will be taken to ensure that confidentiality. Protecting the
participants’ confidentiality in the study is critical to ensure the veterans do not receive any
repercussions that may come with speaking out. Data shall be collected from the VA website
and government resources. The data shall include the number of medical centers, the amount of
money each center uses annually, challenges they face and a comparison between its healthcare
system and a private healthcare system. The challenges faced by veterans while trying to access
15
the VA services shall be collected from the veterans themselves. The veterans are a primary
source of data. The data shall undergo a comparison to find out whether the VA healthcare
prohibits veterans from receiving better services using their private resources. The savings on
government allocations shall also be calculated. The methodological design has the aim of
providing a solution to the existing problem.
Significance of the study
This study is vital both for the U.S. government and the veterans. Veterans play a huge
role in keeping the U.S. safe during their service. Having devoted their lives to serve the
country, it is only fair they are given appropriate medical care whenever they seek for the same
type of security and support, they once gave. In a case where there is a lack of support, the
Veterans Affairs Department offers to the military personnel, then the government can decide to
scale down the budgetary allocations for the department. For the veterans, if the expected output
in terms of services is low, they can decide not to register for VA and seek better healthcare
services privately using their own resources. The study is also vital for the whole country
because when the budgetary allocation for the department gets scaled-down, then the money can
be directed to other vital sectors that shall benefit the U.S. Therefore, the study is significant for
every individual. It is also critical in understanding the various areas where improvements can be
made to ensure the services offered as medical aid to patients who need them are improved to
match the rest offered by the private departments.
Definition of key terms
Veterans Affairs department- A department in the United States of America that takes care of the
military personnel by offering them healthcare services and other benefits.
16
Budgetary allocation- Amount of money the government annually puts aside for a given sector of
the economy. Private Care- Refers to the care that is solicited by veterans themselves from the
private health care facilities using funding from their pockets. Though private care might be
expensive, it is generally available at better quality, as well as obtained on time without cases of
backlog. Quantitative research- Empirical investigation of a specific aspect through the use of
scientific, computational and mathematical techniques. It involves the use of data (Rutberg and
Bouikidis, 2018).
Summary
The study aims to bring out the comparison between the private healthcare sector and VA
healthcare, as far as veterans are concerned. It seeks to gain an understanding as to why many
veterans might opt to go for the private care as opposed to be treated by the VA through
espousing the challenges that they face. There are various research questions hi…
