The EBP Process
EBP is the integration of clinical expertise, patient values, and the best research evidence into the decision
making process for patient care. The first to speak to EBP was Dr. David Sackett in 1996. While his
application was to the field of medicine, it must be applied to nursing in order to manifest the best patient
outcomes.
Clinical expertise refers to the clinician’s cumulated experience, education and clinical skills.
The patient brings to the encounter his or her own personal and unique concerns, expectations, and values.
The best evidence is usually found in clinically relevant research that has been conducted using sound
methodology. (Sackett D, 2002)
.
Your assignment is to post your EBI articles dealing with the identified topic. To assist you with this project,
here is a refresher on the steps of EBP
1
The Steps in the EBP Process
ASSESS the
patient
Start with the patient — a clinical problem or question arises
from the care of the patient
ASK the
question
Construct a well built clinical question derived from the case
ACQUIRE
evidence
the
Select the appropriate resource(s) and conduct a search
the
Appraise that evidence for its validity (closeness to the truth)
and applicability (usefulness in clinical practice)
APPLY: talk
with the patient
Return to the patient — integrate that evidence with
clinical expertise, patient preferences and apply it to practice
Self-evaluation
Evaluate your performance with this patient
APPRAISE
evidence
Steps in Formulating the Clinical or PICO Question
The chart above presented the steps for identifying the BEST practices for
patient care. But you will see that the
of the process is to ASK the
clinical question. We call this the PICO question. Here are the steps to
formulating the perfect PICO question:
Step 1: Formulating a well-built question involves utilizing 4
components:
2
• The patient’s disorder or disease (P)
• The intervention or finding under review (I)
• A comparison intervention (if applicable/not always present) (C)
• The outcome. (O)
Step 2: Identifying articles and other evidence-based resources that
answer the question
Step 3: Critically appraising the evidence to assess its validity
Step 4: Applying the evidence
Step 5: Re-evaluating the application of evidence and areas for
improvement
Once you formulate the PICO question, you are on your way!
Let’s try identifying the PICO question components from this
example:
“In a 45-year-old woman with a 25-year-old history of obesity, would
a behavior modification program with diet and exercise compared to
the use of a diet suppressant drug alone be a better therapy for
resulting in long-term weight loss?”
What is Patient’s disease/problem? _________________________
What is Intervention under review? _________________________
What is the Comparison intervention? _______________________
What is the Outcome? ___________________________________
Answers:(P=obesity, I=diet & exercise, C=appetite suppressant drug therapy, O=long-term weight loss)
If you correctly identified the PICO components, you will have no problems
finding the appropriate articles for your assignment. Happy searching for the
“Evidence.”
