African American Cultural Influence on the Dying Process
The African American cultural values play a major role in end-of-life (EOL), as these inherent values determine the ways in which people construct conventional responses to health, illness, funeral plans and the bereavement process. In order to have positive patient outcomes in end-of-life (EOL) care the patient’s cultural background has to be acknowledged (Kinzbrunner & Policzer, 2011). One aspect of the cultural background of African Americans is that they appear to underutilize hospice care (Yancu, Farmer, Graves, Rhinehardt, & Leahman, 2015). This can stem from their innate mistrust of the US healthcare system because of racism, discrimination and the disparities they currently face today. Another aspect for nurses to consider in this community’s EOL care is their spiritual and religious backgrounds. They believe in life after death and that everyone transitions to the next life. They believe in a God that rules their lives and anything that happens to them is his will. Their end of life planning is directly tied to this, as end of life decisions such as advanced directives, living wills and DNRs can be seen as giving up hope (Kinzbrunner & Policzer, 2011).
As a result of the belief of the African American community, certain factors have to be considered to achieve better patient outcomes at EOL. Due to the innate mistrust of the health care system many African Americans have, the nurse has to establish a trusting relationship. This must be accomplished by ensuring the patients and families that the decisions that are being made are in their best interest (Kinzbrunner & Policzer, 2011). Next, the patients and families view on spirituality should be acknowledged and incorporated in all decisions made and no pressure applied to acknowledge factors they are not willing to accept. Culture has a great impact on how patients and families experience the dying process (Kinzbrunner & Policzer, 2011). Therefore, cultural sensitivity has to be shown towards the family’s belief and recognized in the plan of care.
