Complete the Heritage Assessment, on a non-family member over the age of 45. Create a genogram for your interviewee, including identifying health issues (and age at death and cause of death as appropriate) for each family member to identify patterns and trends.
The genogram can be completed using a free template or you may draw by hand and scanned.
Summarize the findings in a 3-4-page paper, in APA format including the following information:
– conclude health risks
– propose (or develop) health promotion teaching opportunities based on findings.

Introduction- A genogram is an important tool in the form of a pictorial diagram depicting a persons relationship with different members of the family and the medical history and how each of the medical problems is linked with the relationship. It is also known as the family diagram (Butler, 2008). This concept surpasses that of the family tree as it allows the observer to identify various hereditary factors and physiological as well as psychological factors associated with relationship (Jolly, Froom, & Rosen, 1980).
The physiological and physical attributes are being displayed through a comprehensive system of symbols of various shapes and sizes like circle, square, and triangle. Each of the symbols is unique in itself as it associates with gender, a particular condition, relationship, and whether a person is dead or alive.
The circle is associated with female family members; the square is related to male family members, to represent a deceased person a cross is put inside a circle or a square depending upon the gender of the deceased person. A pregnancy is denoted by a triangle and a cross sign inside the triangle denotes miscarriage or abortion.
